Where can I find my policy documents & ID cards?

E-Delivery

If you selected e-delivery on your application, your Dental and Vision policy documents, ID cards, and other details can be accessed through the Ameritas Portal, typically within minutes of enrolling.

If you do not see your documents available by the next day, contact support by your preferred method below.

Physical Copies

If you opted out of e-delivery on your application, your policy document and ID cards will be mailed to you within 7-10 days.
